S. cities, through a 5-year $.4 million MOU collaboration with the U. S. Centers for Disease Control and Prevention (CDC), Bureau of Primary Health Care (BPHC), Community Health Centers (CHC), and the U. S. Surgeon General that brought faith-based collaborations together to reduce health disparities in their local settings, using technological support and reporting systems.
