Participate as a member of the primary/family care team. Provide Primary Care Mangers, PCMs, and their patients with behavioral health consultation services that include follow up consultations. Services were provided for: general behavioral health (insomnia, diet/exercise/weight management, etc); chronic medical conditions (diabetes, chronic pain, etc); mental health (anxiety/stress, depression, grief issues, etc); and psychosocial issues (work related, family, marital, relocation adjustment, reintegration and parenting).
