Daily Duties at Life Changer Health:
Analyze and organize a patient prescription profile. Identify pharmacodynamic/kinetic interactions. Recognize cost saving opportunities. Recommend therapeutic regimen changes to physicians according to clinical guidelines. Educate patient on each disease state, and therapeutic goals & outcomes to strive for and why. Identify barriers to compliance with patients & alleviate with appropriate tools. Distinguish risk vs. benefit. Minimize opportunities of hospitalization or re-admittance. Demonstrate proper use of inhalers, syringe technique and carbohydrate counting. Work with the patients insurance company regarding therapeutic alternatives & prior authorizations. Evaluate patient laboratory values. Document and code all transactions for payment.
