Daily Duties at Superior Health Plan:
Provide care management for 260 Medicaid and Medicare clients. Address their medical needs. Adjust government funds between $79,000 to 120,000, to ensure spending stays within the budgeted amount. Authorize health and dental care. Assist them with obtaining medical equipment and home modifications, as needed. Transition members from a healthcare facility, back into their community. Coordinate with members to do face-to-face visits. Make cold-calls to new members to develop a rapport with them. Work with unhappy members to resolve issues and complaints. Discuss ways to assist members, and alleviate their concerns. Document every contact, made with members, in TruCare system. Support clients not covered by the STAR+PLUS Waiver program with resources. Train two new employees on more efficient ways to use Microsoft Word, Excel, Outlook, and PowerPoint programs—starting with their current knowledge. Network with new and experienced Service Coordinators to learn more efficient ways to use TruCare system.
