DAVITA BAKERSFIELD NORTHEAST IN-CENTER HEMODIALYSIS; Nov 2011-Nov 17 2013 Please, contact Work Number at 1-800-367-2884, Employer Code 11406. 3761 Mall View Rd, Bakersfield, CA 93306 WHITELANE PERITONEAL DIALYSIS AND HHD PROGRAM Nov 2011-Dec 2012 REGISTERED DIETITIAN: I monitored patient’s laboratory values, nutritional status, dietary and medication adherence, response to therapy, and provided medical nutrition therapy, nutritional counseling and education to patients. I provided Vitamin D IV, phosphate binder, and Sensipar recommendations as the Mineral and Bone Disorder Specialist. I was instrumental in improving our quality care of patients to number 1 in our region. I successfully completed the Competency Assessment for Renal Dietitians, participated in IDT Plans of Care and QIFMM meetings.
