Daily Duties at Prime Care Hospice:
Assist Interdisciplinary staff members in recognizing and understanding the significant social and emotional stress factors and/or disorders related to health problems and terminal illness Evaluation of patient/family response to Social Work interventions and assessing caregiver's ability to function adequately Refer patients/families to appropriate community resources Assess the patient/family psychosocial status, potential for risk of suicide and/or abuse or neglect Identifying obstacles to compliance and assisting in understanding goals and interventions
