Daily Duties at Odyssey Hospice:

Responsible for the coordination, direction, and provision of social work services for patients, families, and caregivers Provision of initial comprehensive and continuing psychosocial assessments as it relates to the patient's illness and development of a Social Work Plan of Care, involving the patient/family Participation in weekly Interdisciplinary Team Meetings and case conferences as necessary Compliance with administrative documentation procedures and provision of clinical progress notes/documentation per Hospice policy
