Daily Duties at Bright Health Physicians Of Pih:
•Follow up calls to all transitional IPA patients discharged from the hospital, SNF • Medication reconciliation, follow up doctor appointments, lab, Coumadin Clinic, DME • Authorized and scheduled outpatient classes, diabetic education classes, etc. •In charge of “SNP” program initiated a new patient, made an assessment and continued to make follow up calls as needed to patients for progress update and aide in transportation, provided education and resource materials Durable Power of Attorney Information, life planning resources, etc. This was a continuous process, followed more than 60 patients, until they either moved to a different insurance, or deceased. The goal was to help the patient stay healthy and out of the hospital. • Made CCS California Children’s Services inquiries for authorizations for inpatient stays and approvals for CCS coverage for eligible patients.
