Clinical Oncology Pharmacist at Perelman Center for Advanced Medicine, Hospital of the University of Pennsylvania October 2011 - June 2014 • Practiced in the areas of oncology ambulatory setting including: Lung, Brest, Head and neck, Liquid tumors, Colon, and Skin cancers. • Responsibilities included: review physician orders and dispensing medications with a focus on research oncology protocol orders for ambulatory infusion service. • Reviewed patient specific parameters, verified doses, infusion time and concentrations for IV admixtures and oral chemotherapy, monitored and helped manage drug therapies. • Worked in close contact with primary care providers: MDs, NPs, PAs and investigational nurses. • Worked with investigational protocols for oral and IV medications, followed preparation techniques and dispensing requirements. • Reviewed patient specific parameters and laboratory values for participants in investigation protocols and standard chemotherapy regimens. • Was overseeing pharmacy technicians, internes and students and ensured accuracy of their work prior to dispensing. • Weekends worked in IV admixture room, providing medications for 900 beds hospital. • BCLS certified. Clinical Pharmacist, Hospital of the University of Pennsylvania February 2008 - October 2011 Pharmacist, PharmD • Worked as clinical pharmacist in decentralized setting. • Verified electronically entered orders based on patients weight, height, CrCl, allergies. • Followed patients on pharmacokinetics for antibiotic dosing. • Participated in discharge rounds. • Provided clinical information for medical staff. • Provided counseling for the patients. • Provided therapy recommendations to health care providers (Mds, NPs, PAs) regarding patient dosing based on patient specific parameters, such as CrCl and BSA based medications. • Worked in IV admixture preparation room and supervised teem of technicians. • Monitored and verified accuracy and preparation technics for various IV admixtures, including chemotherapy.
