Daily Duties at Healthy Tomorrows, Llc Dba “arizona House Calls”:
Providing primary medical care for homebound patients with acute and chronic medical problems. Performing physical assessments on patients with histories of chronic and acute illnesses. Providing follow up visits on patient’s status post hospital admission or ER visits. Providing acute care management/plans for patients with acute emergency problems. Chronic care visits for patients with chronic care problems. Duties include ordering, prescribing, and reordering medications, medication and immunization administration both oral and IM as needed. Ordering routine and acute labs, and procedures. Making appointments for patients as needed. Performing vital sign monitoring. Diagnosing new medical conditions, troubleshooting acute medical incidents and patient home safety problems as needed.
