Daily Duties at Huntington Hospital/Senior Care Network:
Initial and annual assessments of clients, homes (physical environments), and resources such as informal support networks (family, friends, neighbors), and those who could be recruited as IHSS providers. Assistance/facilitation with application for IHSS, and discussing with clients/families what to expect in the IHSS application process. Also education of clients/families of available community resources to supplement IHSS and Senior Care Network services, such as respite care, bereavement counseling (for families), Friendly Visitors, safe transportation to medical appointments, etc. I oversaw implementation of durable medical equipment, and safety devices such as bathroom grab bars. Maintained caseload of 42 senior clients, kept expenditures within CDA-approved budget, client contacts by phone or in person minimum of once per month (more if clinically indicated), also assessing for cargiver stress/burnout, instances of suspected elder abuse or neglect (mandated reports to Adult Protective Services). Met on regular basis with supervisors and multi-disciplinary team of RNs and fellow case managers to discuss cases and receive clinical feedback to optimize continuity of care. Maintenance of client records in accordance with HIPAA regulations.
