Daily Duties at Azalaea Woods:

 Assessed facility Medicare residents per Medicare protocol.  Completed Medicare assessments (weekly, monthly- resident specific) as needed.  Completed care plans for every Medicare resident.  Conducted weekly care plan meetings involving resident/family, medical social worker, physical therapist, activity director, and resident nurse.  Conducted department head weekly meeting for Medicare residents.  Assisted Director of Nursing with staffing, nursing and certified nursing assistants’ issues, and DHEC visits and evaluations.  Completed weekly chart audits on Medicare and Medicaid residents.
