Hitchcock Home Health Work Values

Daily Duties at Hitchcock Home Health:

 Conducted weekly patient assessments at patient’s home.  Provided general nursing care (dressing changes, wound vac changes, picc line care, and venipuncture) as needed  Completed patient specific care plans upon admission.  Conducted patient/family education related to specific diagnosis.  Multidisciplinary approach to include- Occupational and Physical Therapy, Medical Social Worker for cumulative patient care.  Reported abnormal findings or significant changes in patient care to Physician.  Served as a liaison between physician and patient/family.
