Daily Duties at Home Care Connections Home Health:
Assessment and data collection on all patients being admitted to home health. Filling out Oasis C, and coding correctly. Planning and Implementation of care, supervision LVN/s and aids. Coordination of care between physicians and other staff, teaching on medications and disease process, IV Therapy, Phlebotomy and wound care.
