Daily Duties at Liberty Hospiral:
Decentralized pharmacist to insure that all health care disciplines work together and realizes that pharmacist is accessible. Review each patient’s medication profile checking for appropriate renal dosing Review appropriate dosing according to manufacturer suggested Pharmacist Consult upon request by physician All Vancomycin dosing, as well as all antibiotic dosing by pharmacist per protocol Aminoglycoside dosing and monitoring Oncology: Review chemotherapy orders, researching protocol to support doses and drugs ordered Insure proper compatibility with base solutions, rate of administration, concentration, length of cycles Sterile and safety technique – preparation and proper handling of chemotherapy in flow-hood Assure proper, safe procedure of chemotherapy Discharge Reconciliation: Hospital adopts the “It Takes Two” approach to discharge medication reconciliation. Pharmacist will be called to double check a nurse. Pharmacist Discharge patient interview: Pharmacist visits with patient or caregiver upon admission and upon discharge
