In this position serving Adult individuals [18 years + older];completed in take & assessment of patients to determine diagnosis and treatment needs: Determined severity of presenting symptoms to assess present level of services required: In-pt, more intensive out-pt- e.g.Out-pt Day program.or entry into Adult Out-Pt program. Intake required interviewing individual for Comprehensive Psych-social assessment, comprehensive Mental Status exam for presentation to Staff Psychiatrist. Responsibility also included obtaining insurance in for and any relevant history or other information from collateral resources: review of any records, info from any family or significant others.
