Daily Duties at The Ruth M. Rothstein Core Center:
Provided case management services in the home for men, women and children disabled by HIV/AIDS. Completed initial intake/assessment of current needs and biannual reassessments. Assisted clients and their families with obtaining services that allowed them to live independently in their own homes. Advocated on client’s behalf in obtaining various benefits including food, financial assistance, housing subsidies, transportation, home healthcare etc. Made referrals to external agencies and coordinated services in the transfer of a client to hospice or nursing home when client was no longer able to live independently. Acted as team leader and liaison between management and ORS program.
