Daily Duties at Asheville Specialty Hospital :
Able to establish a rapport with patients and families to define a plan of care. Assist clients to obtain information from all available care providers, throughout all service lines to achieve plan of care goals. Lead team meetings, to include all disciplines, to accomplish plan of care goals. Facilitate discharge process for patients who may reside in WNC, Ga, SC, Tn and coordinate needed equipment, services. Set up all follow up appointments with physicians. Assist insurance companies with clinical updates. Work within a budget to achieve a positive outcome for a caseload of up to 17 complex patients.
