Performed discharge planning and education for the patients. Provided in-services and oversaw medication passes to nursing. Served as a preceptor for pharmacy students on rotation. Insured CMS core measures were met, performed Pharmacokinetic Dosing and Heparin Management. Performed computer order entry of medication orders.
