Provided consultative services and made recommendations for therapy changes to the physician as appropriate. Attended Discharge Planning, Quality Assurance and Performance Improvement meetings. Performed discharge planning and education for the patients. Provided in-services and oversaw medication passes to nursing. Served as a preceptor for pharmacy students on rotation. Insured CMS core measures were met, performed Pharmacokinetic Dosing and Heparin Management. Performed computer order entry of medication orders.
