Daily Duties at Vans Institutional Pharmacy:
Perform review of patient medical charts for Intermediate Care/Mentally Retarded Facilities. Review adverse drug reactions, drug-drug interactions, drug disease interactions, therapeutic duplication, contraindications and lab monitoring. Report findings and recommendations to the Attending Physician and Director of Nursing
