Daily Duties at Nacogdoches Memorial Hospital:

• Focus on reducing Traditional Medicare 30-day readmissions by coordinating care with an interdisciplinary team approach to improve the care transition from hospital to home or post acute care. • Serve as a liaison between hospital staff and the patient and/or family. • Provide education and guidance to the patient and/or family for an effective care transition, improve self-management of care and enhanced patient-provider communication. • Provide detailed specific education over medications, diagnoses, co-morbidities, diet, needs prior to discharge, family support, physical limitations with self care, need for medication assistance, need for transportation assistance, need for in-home services, and post discharge placement. Work closely with physicians, social services, nursing staff, and case management to ensure arrangements are made prior to discharge if needed. • Liaison between hospital staff and downstream providers. • Identify high risk patients and notify staff. • Identify readmissions, notify staff and conduct a root cause analysis for each. • Follow-up with patients post discharge to insure follow-up appointments are scheduled, address any questions or concerns, notify patient/family/downstream provider of necessary follow-up labs/testing, and address patient’s health status. • Provide reports and data related to Traditional Medicare readmissions.
