Daily Duties at Northern Riverview Healthcare Center:

Social Worker Initially I was hired for the Alzheimer's unit; I progressed to cover multiple areas of the facility. I delivered holistic services in individual, family, and group settings, supporting independence and ensuring well-being of all residents from admission to discharge. Conducted psychosocial assessments, created/discussed treatment plans with IDT, and evaluated effectiveness. Performed ongoing case management duties including resident advocacy, liaising regularly between residents, medical team members, and community resources. Made referrals for home care and hospice, ordered medical equipment, and secured transportation. Provided grief and loss counseling as well as crisis intervention. Scheduled family meetings and attended monthly resident council gatherings at request. Educated residents on upcoming elections and helped with voting. Informed residents of their Personal Needs Account (PNA) statues, rights, and privacy policies on an annual basis. Covered for supervisor during 3-month absence, overseeing all department operations independently and training a temporary assistant; ensured zero deficiencies, confirmed all progress notes as well as MDS3.0 records were complete, and readily took on added responsibilities including cleaning storage closet. Designed/implemented new assessment strategy for interviewing residents. Constantly monitored any changes resident's moods/behaviors. In addition I monitored changes in their psychotropic medication when made by the psychiatrist as it may relate to changes in mood/behavior. I worked closely with the IDT team and families to help residents with mood/behavioral issues. I was in regular communication with the psychologist. I utilized depression assessment tool as needed to monitor resident's psychosocial well-being, notified all appropriate IDT members of issues requiring attention. Partnered with Volunteer Ombudsman to aid with resolving resident/family complaints and requests. Responsible for effectuating the change of resident's physician per request of the resident or responsible party. Cultivated and nurtured positive, productive relationships with residents, families, health team members, and community providers to promote effective coordination of resources.
