Daily Duties at Nurses Registry Home Health:

Evaluated/admitted/ annually recertified (readmitted) patients for (as applicable) personal care, housekeeping, adult day care, caretaker respite care. Covered 16 county territory, made regularly scheduled home visits. Collaborated with inter and outer agency personnel to ensure patient access to medical supplies & equipment, and other needed services (i.e. transportation). Supervised work of home health aides. Goal of waiver program was to keep patient at home as opposed to being placed in a long term care facility. Also had to recognize - with ongoing evaluations/visits - if patient could no longer be safely maintained in home. Assisted patients' families, physicians, and social services with this. Medicaid regulations utilized with all phases of care.
