I utilized process of assessments, planning, facilitation and advocacy for options and services to meet patients needs while promoting optimal, cost effective outcomes.Working with MD's to get HHC and dme ordered along with any other needs patient needed when p dc'd to provide for safe dc home. I did presentations at MD office regarding agency and different diagnosis's and programs we had to offer to care for his patient. I identified opportunities to enhance clinical engagement with the physicians group's member's in order to improve quality ,patient experience, and efficiency for both Medicare and Commercial patients in the local geography. I ensured compliance with all state and federal regulations as well as Corporate guidelines. Worked with staff in hospital to help them with dcp. I covered two hospitals with about 100 patients in each hospital. Monthly reports to senior vp regarding productivity
