Daily Duties at Odyssey Hospice:

Direct patient care which varied based on individual needs of the patient. Coordinated all of my patients care with other interdisciplinary team members. Implement patient plans of care,updating as needs change as well as ensuring that other team members carry out their portion of the plan of care. Throughout the hospice journey, I continually monitor the patient's symptoms, and with the help of the hospice Physician,we accommodate their symptom changes and needs.Make sure all patients medications are readily available and ordered as needed.Hospice nurses aid assignments are individualized for each patient and changes are made as a patients ADL needs change.
