Daily Duties at Northport Medical Center:

Worked from May 2003 till November 2003 in the Emergency Room as the Intake and Referral Coordinator. Worked 4-12pm seven days on and seven off. Saw any psychiatric cases that came into the ER. Did psychosocial assessments, consulted with psychiatrist on call or their personal psychiatrist if appropriate. Determined appropriate placement and assisted with placing the patient either in North Harbor, or transferring them elsewhere. If being discharged, assisted with follow-up care. In November 2003 was transferred to North Harbor full time as one of the four inpatient social workers. Did psychosocial assessments with patients on admission, wrote treatment plans with an interdisciplinary treatment team including fellow social workers, doctors, psychologists, and nurses. Responsible for collateral contact with family and family meetings to determine discharge possibilities. Met daily with the team to discuss progress. Assisted with discharge plans, transportation, etc. Did educational groups for the Adult I and Adult II units. Individual and family counseling. Wrote commitment paperwork and filed with probate court upon doctor order. Called to testify in multiple commitment hearings. Worked with nursing homes for placement of geriatric patients when appropriate. Did MMSE testing with primarily geriatric patients to test for memory deficits, possible dementia or Alzheimer's.
