Daily Duties at Community Based Warrior Transition Unit:
Nurse Case Manager (NCM) for Soldiers in the Community Based Warrior Transition Unit-AR (CBWTU-AR); Manages up to 20 Soldiers from all COMPOs and multi-state areas; utilizes full spectrum NCM roles to meet complex health needs of remotely managed CBWTU population; incorporates team approach, facilitates multi-disciplinary involvement, including Soldiers and families, to achieve goals of Comprehensive Transition Plan (CTP) and reach Medical Retention Determination Point (MRDP); initiates/manages appropriate, timely and clinically effective medical care through TRICARE, military, and VA providers; addresses problems at all levels to smooth transition through program; meets access standards/NCM documentation standards; participates in activities to develop officer/professional skills; fosters unit cohesion.
