Daily Duties at Vitas Innovative Hospice Care:

I manage a 14 bed inpatient unit through interdisciplinary team collaboration, provide diagnostic evaluations of the psychosocial problems of the patient/family unit, define social service goals for patients and their families, provide counseling and casework needed to social service goals, develop and execute plans of care, maintain psychosocial documentation, provide in-service training, develop and coordinate relations between Hospice and outside medical/health services, peer support and counseling, facilitate bereavement groups, individual, group and family counseling, crisis intervention, provide supervision, provide field education to MSW student interns, establish and maintain vendor relations, develop, initiate and execute discharge planning.
