Daily Duties at North Bay Healthcare:
Case Management responsibilities include the assessment of all patients admitted to the Medical/Surgical unit to ensure that their healthcare needs after discharge are met. Complete referrals to ancillary staff in the hospital as needed. Maintain open channels of communication between the patient, family, physician, and nurses to ensure that the appropriate level of care for the patient is met. Complete formal discharge plans on identified at risk patients. Complete concurrent reviews and communicate with insurance providers to ensure that appropriate acuities are met.
