Daily Duties at Grande Prairie Heath & Rehab:
Leadership o Formulated and implement the individual care plans used for patients and supporting documentation. o Utilized the data from assessments to include support from social services, dieticians, rehab specialists, and medical staff to meet the patient plan of care. Financial Stewardship o Completed Resident Assessment Instruments (RAIs) used for reimbursement from Medicare and Medicaid patients in rehabilitation and long term care setting. Train other departmental staff on raising awareness and knowledge of financial management matters. o Ensured that the plan of care is consistent with ethical standards and Medicare requirements. Clinical Management o Assessed the functional capabilities of residents of certified nursing facilities. o Conducted assessment of the resident nutritional requirements, cognitive ability, and behavior patterns, hospitalizations, and mood and obtain and account for documentation to support patient condition. o Utilized assessment results to formulate and implement individual care plans for the residents. o Implemented and monitored these care plans to ensure effective patient outcomes.
