Daily Duties at Mhm Services, Inc:

Coordinated with medical staff, the delivery of behavioral health patient care to substance abuse clients through the process of assessment, planning, implementation, and evaluation. Led a grief support group for clients struggling with the loss of loved ones. Implemented discharge planning to ensure continuity of care with community behavioral health services and Social Security Agency (for SSI and SSDI claims). Worked with forensic supervisor to provide discharge planning for a Deaf inmate. Served as witness in court for a 303 Commitment Hearing. Supervised Bachelor Social Workers (BSWs) and Master of Social Work (MSW) Interns from the University of Pennsylvania. Led Warden’s Special Needs Treatment Team meetings.
