Daily Duties at United Healthcare:
Telephonic and on-the-ground case management for 50 members in the Advanced Illness program. Performed nursing assessments, facilitated referrals for hospice evaluation, DME or social services intervention as needed. Performed patient and family teaching related to patient's disease.
