Daily Duties at Northeast Georgia Medical Center:
Helped open a 9 bed CVICU for a new program for cardiothoracic surgeries for the above hospital. Recovery of fresh post op CABG and/or valve surgeries from the operating room. Connect all PA catheter lines to appropriate equipment, arterial lines, Subclavian lines, expertise in all critical drugs--examples--Epinephrine drips, Norepinephrine, Nipride, and many other IV drips. Had to know which one to use at what time. It was all up to the nurse taking care of the critical patient. Patient would arrive with 3 chest tubes, on ventilator, Diprivan Drips for sedation, and many drips. All arrived on Insulin Drip with every 1 hour checks and titration of the Insulin Drip. Following protocols for Potassium and Magnesium replacements. Watch for excessive blood loss from chest tubes, all patients were atrial and ventricular paced, watching intake and output, assessing constantly all cardiac parameters. It was a one on one patient for first 8 hours. Weaned off of ventilator at appropriate time according to blood gases, chest tube drainage, etc. Day 1 getting patient out of bed, walking, removing all PA lines and arterial line if patient was stable and off all vasopressive drugs. We as the nurses were trained by the surgeons to remove all chest tubes at one time. Day 1 you would have 2:1 ratio. Had to have excellent critical care skills as well as knowledge in hemodynamics. Able to look at X-Rays to assess for pleural effusions. Also had advanced skills in cardiac rhythms and arrhythmias and what to do if they develop.
