Daily Duties at Cleveland Regional Medical Center:
I worked 12 hour day shifts with 3 other nurses in this mixed ICU/CCU unit. This rotation required me to also work every other week-end. We each would have 1-2 patients depending on the acuity of the patients. Our patients were a mix of critical cardiac, surgical and medical patients, often admitted from the emergency room. Once recieved we would continue stabilization of the patient by using our critical care training skills to assess, implement and innate care based on assessment priorites. We worked closely with physicians or cardiologist to stabilize the patient. These were critical patients that were on ventilators, and had multiple IV medications and treatments that needed critical skills to manage. The ICU nurse has to be able to assess all body systems and assist in the treatment plan by continually monitoring critical lab values, EKG"s neurological and hemodynamic states of the critical patient. Acute changes needed early detection by the acute care nurse so that the physician could be notified and treatment given based on the nurses assessment skills. This ongoing assessment and intervention was often done on more that one acute patient at a time. The average patient ratio was 2 patients for one nurse. We sometimes had an aide, and if not critical care nurses need to work closely together in such a small unit and pull together as a team to optimize good patient outcomes.
