Daily Duties at Cleveland Regional Medical Center:
Assess and intervene emergency patients. Multiple system assessments, stabilization and work closely with the emergency room physician to identify disease process and stabilize the patient. I worked the day and afternoon shift. I would work triage at times,( quick assessment pt when they first came to the ER which was a brief history and vital signs), and transition patients according to acuity to a part of the ER based on the assessment. I only did this on occasion. Most of the time I actually worked int the emergency. After triage, the patient would come back to us and I would do an more detailed assessment of the patient and begin appropriate intervention if the ER physicians were with other patients. It was an approximately 16 bed ER with different levels of trauma areas.(The patient was sent to an appropriate trauma level based on the triage assessment). I worked with several other nurses and we each would have 3 or 4 rooms with patients of different acuity. Anywhere from a small laceration that would need sutures, a injured limb that needed x-ray'd and then casted or immobilized, to an acute trauma patient that I would work with the ER doctor and other skills to stablize and then transport to the ICU unit in the hospital. These patients may require immediate intervention for various types of shock that may of resulted from blood loss, acute cardiac event (heart attack) or sepsis (acute systemic infection). This immediate intervention may be fluids, IV drip meds that needed initiated and managed by titration for blood pressure or heart rate, or patient presenting with a stroke that needed TPA (clot buster medication). I worked a 12 hour shift in this ER unit. At the time i did have my PALS (pediatric advanced life support certification), and did take care of multiple pediatric patients along with the ER doctor, though that is not my area of expertise.
