Daily Duties at Visiting Nurse & Hospice Of Vt & Nh:
*Build awareness of hospice and homecare services in southern Vermont & New Hampshire. *Increase referrals through relationship building with physician offices, hospitals, skilled nursing facilities and assisted living facilities. *Provide ongoing customer service through follow up with Directors of Nursing, discharge planners, social workers and administrators at all levels, and by working closely with VNAVNH clinical staff to resolve issues of communication and continuity of care. *Presentations to and networking with professional and community groups on the benefits of hospice care, agency initiatives and the “Start the Conversation” campaign to raise awareness of end of life choices. *Participate in complex disease pilot programs with area hospital and nursing homes to reduce patient readmission. *Facilitate case reviews with all physician groups and VNAVNH Community Nurse Liaison teammate.
