Daily Duties at Yai:
Work with adults with mild to moderate developmental and learning disabilities in a day habilitation and community setting. Provide pre-vocational training at local community volunteer sites. Network and partner with other local community sites for future use in client trainings. Help clients develop personal awareness skills, independence and assist in providing community access, while being a liaison within the community. Meet with staff members and client’s family to create an Individualized Service Plan. Assess client’s needs to create and write individual goal. Keep data collection on clients behavioral, social, and academic goals. Schedule client vocational and recreational activities. Counsel clients on personal concerns and during crisis situations. Teach adults with developmental disabilities personal awareness, provide community access, and serve as a liaison with the community. Work closely and coordinate schedules with families, residences, and case managers. Communicate clearly and effectively with families, case managers and/or residential staff. Interviewed and screened potential clients for Day Hab Program during a weeks time to determine if our services would meet clients needs. Informed families about our program and services we provide. Gave families tours of our site and taught them about different sites we volunteer at. Counseled families about how to deal with behavior issues at home. Counseled families about services the client was receiving and other services that might benefit the client. Prepared documents for client to do vocational work at volunteer sites. Took data on clients progression on individualized goals. Used data to drive individual goals on Individualized Service Plan (ISP). Develop ISP based on data collection. Communication skills -collaborated with administration, psychologist, psychiatrist, social workers, families, and volunteer site staff to ensure client was provided appropriate services and needs were met. Advocated for clients needs and services. Provided training to new staff about crisis behavior management, data collection, and counseling strategies. Maintain client records database, services and case notes. Coordinate schedules with day hab, residence, case managers, and volunteer sites.
