Iserved as the charge nurse with many hands on duties as well.I did admissions and assessments of incoming new residents post hospitalization-included care planning with all departments,MD,resident and family if required-administered medications-tube feedings,IV therapy,wound care,infection control,inservices with staff,coordinated with hospice and there services for residents and families. Finally a safe discharge plan back to there independent setting at Abbey Delray or other previous home settings.
