Daily Duties at United Health Care Military And Veterans:
provide consistent process for the UHC M&V staff to assess and identify opportunities and /or gaps/barriers in order to develop a beneficiary centric care plan . The beneficiary is monitored for the effectiveness of the care plan and the plan is modified as needed with collaboration between the beneficiary, the clinician and other members of the health care team. Case management is a process designed to assess, plan, implement, coordinate, monitor, and evaluate the options and services necessary to meet an individual’s health care needs. Using communication and available resources to promote quality, cost-effective outcomes, case managers work one-on-one with the providers. Act as beneficiary advocates, working with multidisciplinary teams using clinical skills and knowledge to assure that the best possible care is provided.
