I have a strong ability to work without direct supervision required. Excellent communication skills preferred. Knowledge of nursing principles and practices required. Knowledge of the nursing process and its applications to patient care required. I can provide care appropriate to the age of patients served on the job. I am able to develop skills in use of computerized information/Misys system used in home care setting. I have excellent documentation skills required. I believe I am strong in customer service skills to coordinate service delivery including attention to customers, sensitivity to issues, proactive identification and resolution of issues to promote positive outcomes for members. I can good computer skill and can navigate computer system through internal/external computer systems, including Excel and Microsoft Word. I have the ability to effectively participate in a multidisciplinary team including internal and external participants.
