Delivering care at 22 beds skiiled nursing care facility.Assess patient status,and notify physician,prepares patient's admissions and discharges,Interact with departments regarding patient care.Educate patient/families on health care needs,conditions,options.perform medical procedures such as dressing wound change,starting intravenous lines to deliver antibiotics for extreme infections.Identifies and prioritize actions based upon pt.care requirements.Follows infection control procedures.Create care plans for residents and evaluate effectiveness of interventions.
