Daily Duties at Lake Forest Place:

Delivering care at 25 beds skilled nursing care facility at Sub-acute care unit.Collaborates with physician or other Licensed Independent practioner to arrange for further medical treatment when needed.Documents accurately all elements of nursing assessment,treatments, medications administered,discharge instructions provided and follow up care arranged.Evaluates health needsof patient's and provides necessary coaching and instruction.Identifies emergency situations and provides care within legal scope of practice.Administers medications and medical treatments completetly as directed by a physician or licensed Independent Practioner.Maintains medical records (In some health centers it may be using electronic medical record software.Follows infection control procedures.Maintains patient privacy and confidential information.Knowledge of Internet software,word processing software.Create careplans for residents and evaluate effectiveness of interventions.Perform medical procedures,such as changing wound dressings, central venous catheter dressing change and care,Provide enteral feeding,foley catheter care,or starting intravenous lines to deliver antibiotics for extreme infections.Maintaining updating patient records and operating medical equipment.provide emotional support to patients and their families as well.Identifies and prioritize actions based upon patient care requirements.Provides a safe,comfortable,therapeutic patient enviroment.Revises the plan of care and effective/innefective nursing interventions.Uses clinical judgement in evaluation activities to meet the patient care need of an assigned unit/floor including establishing priorities.Help physician's during exams and treatments.Provided supervision to the certified nursing assistant.Assisted in the orientation of new staff.
