Daily Duties at Focus Home Health Care,Inc:
Makes initial evaluation visit,regularly evaluates the patient's nursing needs.initiates the plan of care and necessary revisions.Initiates appropriate preventive and rehabilitative nursing procedures.Prepares clinical and progress notes,Coordinates services,informs the physician and other personnel of changes in the patient's condition and needs,Counsels the patient and family in meeting nursing and related needs.Participates in team meetings and clarifies individualized reporting responsibilities with all new assignments.Maintain client records showing systematic assessment,planning,intervention and evaluation.Initiates actions to improve the quality patient care.takes appropriate actions independently and responsively in home care situations.Promotes personal safety and a safe enviroment for clients and co workers.Demonstrates knowledge of safety and a safe enviroment for clients and co workers.Demonstrates safe practice in the use of equipment.Assess enviromental safety and takes initiatives to prevent accidents and promote safety.Demonstrate competency in areas of assigment or identifies the need for additional training.(specialized equipment)
