Home/Health Hospice Nurse – Assessed patients for need of skilled nursing services in their homes, provided dressing changes, venipunctures, IV therapy, suprapubic and foley catheter changes, and other procedures, communicated with physician and took orders, computer documentation of patient assessments, performed OASIS assessments for Medicare, long-term care assessments for insurances, taught patients and their caregivers about disease processes, procedures, medication regimen, etc. Quality Improvement and Assurance Nurse – Research best practices, develop staff and patient education material; perform in-services about statistics received from Medicare and the state about the new procedures and protocols developed for best practice. Monitor agency’s improvement and analyze the chart audits performed to assess agency’s performance according to best practice, input chart audits into Excel program to generate reports for the agency and report to the DON.
