Requires good communication skills and ability to build a trusting relationship with patients who may feel vulnerable in their own homes, having lost the mobility they had prior to the most recent hospitalization. Evaluate orthopedic patients on return home from the hospital (total hip replacement, knee replacement). Make recommendation for safety and falls prevention in the home; review total hip and total knee precautions. Provide wheelchair training, bed/toilet and shower transfer training, kitchen training. Make recommendations for durable medical equipment and follow up with equipment training. Help establish ADL routines (Activities of Daily Living), and train families, and home health aides in safety routines, falls prevention for their loved ones.
