Daily Duties at Outreach Pt/Ot/Speech Rehabiliation, Midpoint, Mitchell/Martin Inc., Home Care/Contract Therapist (9/2010 3/2011):

Evaluate and treat patients recovering from orthopedic surgery, neurological or cardiac surgery or chronic conditions such as dementia, arthritis and falls. The aim of therapy is to evaluate and establish realistic goals and implement a treatment plan that meets the needs of each patient in their home. Helped train the families and home health aides in safety and exercise routines for their loved ones. Provide in-home wheelchair evaluations, durable medical equipment recommendations and discharge planning. Forge relationships with agency staff, therapists, nurses, patients and their families and equipment providers. Demonstrate excellent communication and interpersonal skills with a results driven approach. Completed documentation according to agency standards and comprehensive evaluations (including ADL status, upper extremity range of motion, strength, cognitive level, fine motor status, transfers, safety) assess durable medical equipment needs, and follow up with patient, home health aide and family training for DME purchases. Communicated with doctors and their staff for specific orders as needed. Contract work with Mitchell Martin consisted of a short-term assignment, at Seacrest Nursing Home, subacute care, Brooklyn New York. Main responsibility was to perform occupational therapy evaluations (similar to evaluation duties at Jewish Home and Hospital).
