Daily Duties at Ingalls Home Care:
Home Health Nurse/Case Manager • Case Manager duties include but are not limited to general assessment of clients needs, coordinate care of patient from admission to discharge • Instructions on disease process, medications, nutritional guidelines, performed various procedures according to Physician orders and supervision of Home Health Aide. • Confer with Physicians and other disciplines to make sure goals and needs of patients are met • On Call Duty assist of triage calls and instruct on specific needs of patients. • Knowledgeable of Medicate Regulations and Guidelines • Patients treated with respect, dignity, privacy and confidentiality maintained.
