Daily Duties at Heartland Hospice:

Clinical Supervisor with rotating administrative and triage duties for 150 hospice patients; responsible for regulatory oversight of team documentation of care, including admission, comprehensive assessments, clinical documentation, transfers and end of care. Collaborated with Director of Professional Services to ensure quality control and performance improvement. Provided staff supervision and served as team leader for RN Case Managers, RN per diem staff, LVNs, social workers, and spiritual care coordinators for two hospice teams with 90 patients, including IDT, regulatory documentation, and team coordination.
