Daily Duties at Dr. Albert Bayer:
- Manage patient treatment and care of in and out patients with various degrees of brain injuries/disease processes of organic origin, and auto accident related injuries. Attend and facilitate medical appointments, physical therapy appointments including all follow up testing and treatment. A personal advocate and case manager, promoting stable health in the home, out of hospital care. Reducing injury due to negligent home provisions, patient non-compliance of medications, and general supervision of health care needs, patient compliance of medical recommendations, and personalized professional support to maintain the health of the participants. Create and initiate care plans, home assessments and implementation of Home Care needs. Manage new patient intake as well as patient discharge. Case-by-case basis. Very successful in meeting goals and maintaining participant independent living status in their home environment. Case Manager RN.
