Daily Duties at Midstate Medical Center:

Develop and implement a plan for each patient to assure timely care. Collaborate with MD’s to facilitate timely tests, treatments, ensure compliance with quality patient care regulations and measures. Discharge planning to assure the patient progresses through the continuum of care and is discharged appropriately. Conducts reviews using specific indicators and criteria on going from admission to discharge. Frequent communications with physicians to maintain appropriate costs per case and desired patient outcomes. Assess patients progress through expected hospital course. Facilitate healthcare rounds with the healthcare team daily to assess hospital course and determine discharge needs. Meet with patients and families to determine home care needs and services. Serve as patient advocate and collaborate with medical provider, nursing staff and others in the healthcare team to provide for a safe discharge to home. Arrange with home care agencies to provided needs services at home. Provide for needed durable medical equipment at home. Council and teach patient and family home care. Resource person for many different patient needs.
